Most PPO Anthem, Blue Cross\/Blue Shield, Cigna, United and VA CCN insurance plans, as well as plans that utilize MultiPlan and PNOA, are accepted.<\/p><p>Easily check coverage below by completing the 2-minute online form.\u00a0<strong>If covered, you\u2019ll have as many appointments as necessary at no cost to you!* <\/strong>Medicaid coverage is also accepted and provides full coverage for lactation services. To obtain no-cost care, simply indicate you have Medicaid on the Request a Consultation form.<\/p><p>*For in-home consultations covered by commercial insurance, a fee of $50 applies for locations within a 30-minute drive from my office, and $75 for travel beyond 30 minutes. If you don\u2019t qualify for upfront insurance coverage, I\u2019ll provide you with a superbill that you can submit to your insurance for reimbursement. Lactation consulting services are eligible for reimbursement through a flexible spending account (FSA), health savings account (HSA) or health reimbursement arrangement (HRA).
